The service will provide 10 residential care places across a minimum of four locations at any one time, for children and young people with complex support needs. The service model will include a minimum of one worker to two children and young people at all times, across each location, that includes awake support 16 hours per day, plus an 8-hour sleepover shift, 7 days per week. Children and young people may not always require worker support, for example during school hours, while at other times, children and young people may require worker support from more than one worker, for example during a transition or acute period. The service will provide care, which ensures children and young people in and/or leaving residential care placement services have improved personal safety, cultural identity and connections, social and community connections, life skills, stable and secure living arrangements, education and/or employment opportunities and access to services. For children and young people placed within non-family based services, the goal is to work towards achieving the most appropriate permanency option, either family reunification or an alternative long-term care option. Residential care services are required to partner and build linkages with Government and non-government stakeholders, including Child Safety Service Centres, to support coordinated needs-based intervention. The service and the department will work collaboratively to develop a Reporting for Outcomes report, which will include qualitative information and outcome data. The service will complete and submit the agreed upon report as an attachment to the Performance Measurement Data requirement specified in Section 7.2. The service must provide culturally safe services to Aboriginal and/or Torres Strait Islander children and young people to ensure their cultural identity and connection to family, kin, culture and Community is assisted, supported and maintained. The service must be able to facilitate meaningful engagement that recognises the diversity within cultural and ethnic groups. The service may be required to support children and young people with disability support needs and to work collaboratively with other service providers, as required, to implement any National Disability Insurance Scheme plans. The service must be responsive to the department’s regional processes for managing and responding to localised service demand. Placement of children and young people will be made in accordance with the matching processes agreed between the organisation and the department. This matching process will take into consideration the needs of any children and young people in existing placements, including appropriate safety and risk assessments. In some emergent circumstances, a same-day placement for children and young people referred may be required. Any placement of a child under the age of 12 will be mutually agreed to by both parties before the placement commences. One of the premises from which this service operates is a departmentally owned property under a lease arrangement. In lieu of annual rent, we will withhold an approved amount of $40,140 per annum (indexed annually), which is not included in the funding amounts detailed in Items 5 of this funding schedule.
